The Ramsay Home Health team is made up of a wide range of health professionals and support staff, dedicated to ensuring you receive the care you need in the comfort and safety of your own home.
Ramsay Home Health clinical care coordinators
Ramsay Home Health clinical care coordinators are experienced nurses or allied health professionals. They provide an important link between the patient, their doctors and Ramsay hospitals and oversee the implementation of each patient’s care plan. This enables the team to deliver the highest quality care while the patient transitions from hospital back to life at home.
Specialist nurses
Patient care is delivered by fully qualified, experienced nurses. Virtual Heart Failure Service patients are supported by a dedicated team of specialised cardiac nurses.
Allied health professionals
Our community-based teams deliver individualised programs and include allied health professionals such as dietitians, physiotherapists and occupational therapists. Ramsay Home Health also coordinates other support services that may be needed during a patient's recovery, such as domestic cleaning or meal delivery.
If the Ramsay Home Health community-based team is unable to provide a service that a patient needs, we can draw on our extensive network of external community-based providers.
All of our community-based team members undertake thorough credentialing and are required to meet stringent quality standards. All Ramsay Home Health clinical care services are overseen by a doctor.
