It's true what they say: there’s no place like home. That’s why we’ve developed our Hospital Care at Home program – to help you leave hospital sooner while still receiving the care and support you need, in the comfort of your own home.
What is Hospital Care at Home?
No-one wants to stay in hospital longer than they need to. If you're eligible, with Hospital Care at Home you can choose to receive your care at home.
If you and your doctor agree that Hospital Care at Home is a good option for you, a health care professional or doctor at your hospital will make a referral to Ramsay Home Health on your behalf.
A dedicated Ramsay Home Health clinical care coordinator will contact you to discuss your specific care needs and help you check if you're eligible for program funding. Your clinical care coordinator will also work with your care team or doctor to create your tailored Hospital Care at Home program.
Your program may include:
- Nursing – To help you with wound care and healing, intravenous antibiotics, stoma or drain management, and overall health assessment
- Allied health, meals, personal care and home care – If your care team feels you need additional support.
The Hospital Care at Home program does not arrange for at-home doctor visits. However, your clinical care coordinator will keep your doctor or surgeon informed of your progress.
