How to refer a patient
1. Discuss Ramsay Home Health with your patient
Talk with your patient or client about the program you think could benefit them (Rehab at Home, Hospital Care at Home, or the Virtual Heart Failure Service).
To be eligible for Rehab at Home or Hospital Care at Home, your patient must:
- Have a condition that has resulted in impairments with associated functional loss (for example: mobility, balance, safety, ADLs and/or exercise tolerance)
- Have a condition that is likely to respond to rehabilitation within a reasonable/appropriate time frame
- Be able to actively participate in rehabilitation (both physically and psychologically)
- Have a rehab discharge plan with clear goals and anticipated functional gain
- Be medically stable and able to participate.
A patient may be ineligible for Rehab at Home or Hospital Care at Home if:
- They require 24-hour nursing care
- Their home is unsafe for health professionals to attend
- Their cognitive status limits their ability to safely and actively engage in the program
- They have medication or substance abuse problems
- They are receiving end-stage palliative care (EOL)
- They are under 18 years of age.
2. Submit a referral
To refer a patient, first download the appropriate form at the bottom of this page. Complete it and send it to the relevant email address.

3. Await confirmation
If you have referred a patient to Rehab at Home, Hospital Care at Home or the Virtual Heart Failure Service, you should receive an email acknowledging your referral within two to four hours. If you do not receive this email, please call our team so that they can follow up for you. Call 1800 799 732 for Rehab at Home or Hospital Care at Home, or 1300 131 186 for the Virtual Heart Failure Service.
Receiving this email does not mean that your referral has been accepted, as the team will first need to confirm your patients’ funding and clinical eligibility. You will receive another email once this has been done.
Getting health fund approval usually takes less than 24 hours. However, sometimes the process can take up to seven days, so it is important to send your referral through as early as clinically appropriate to allow time for this. Our clinical care coordinators will keep you and your patient informed throughout this process.
